Located in one of the fastest growing suburbs in Northeast Wisconsin, this program offers an accessible space with several breakout rooms for individuals with developmental disabilities with significant behavioral support needs. Social integration and community involvement are a main focus at this site.

- One to one ratio
- Transportation
- Paragon Activity Club - After School Respite
- Health and personal needs include but are not limited to:
- Ambulation support (i.e. wheelchairs, walkers, canes)
- Incontinence care (i.e. depends, toileting schedules)
- Catheterization
- Diabetes care
- Special feeding assistance (i.e., g-tube, one on one feeding, choking prevention)
- Seizure support (vagus nerve stimulators, PRN protocols)
- Other conditions requiring daily medication administration
Staff to person ratio is 1 staff to 3.5 people.
