Plymouth is one of the fastest growing suburbs in Sheboygan County and our Program is easily accessible being located just south of Highway 23 and at the corner of Route 57 & County Road C. This program offers an accessible space with several breakout rooms for individuals with developmental disabilities and brain injuries. Some program areas have yet to evolve and will be established based on the needs of the individuals we serve.

- Adult Day Programs:
- Supporting all program types, program evolving
- AM/PM Respite
- One to one ratio
- Transportation
- Health and personal needs include but are not limited to:
- Ambulation support (i.e. wheelchairs, walkers, canes)
- Incontinence care (i.e. depends, toileting schedules)
- Catheterization
- Diabetes care
- Special feeding assistance (i.e., g-tube, one on one feeding, choking prevention)
- Seizure support (vagus nerve stimulators, PRN protocols)
- Other conditions requiring daily medication administration
Staff to person ratio is 1 staff to 3.5 people.
