As you consider our program, these forms are the first step in our application process. Please be assured that all information is confidential and never shared without your permission. *

To request an Assessment for determination of admission, fill out the enclosed forms and send them to the P.A.C. office. The Program Director or designee will contact you promptly to set up a convenient assessment date. The assessment generally takes place in the individual's primary residence and involves a detailed inquiry into the individual's needs and wants. Please set aside approximately two hours for this assessment. A decision is typically made and shared with the applicant at the end of the meeting. Please see the Caregiver Packet for details on the admissions process.

Once accepted into the program, a non-refundable $40 admission fee is required. Upon completion of the applicable paperwork and bank approval, dates of enrollment can be reserved.

The Caregiver Packet is designed to answer the most commonly asked questions regarding our philosophy, policies and procedures.
