Reporting and Recording RequirementsAt the time of admissions planning, an Individual Support Plan is developed, based on the initial assessment for services. After admission, formal six month and annual reviews are scheduled which involve the individual receiving services and/or guardian, care manager, approved family members/advocates, the residential provider (if there is one) and the Paragon Program Coordinator or designee. These reviews serve as progress reports wherein progress is presented and agreed-upon goals and objectives are reviewed and updated. Goals and objectives may be updated, modified, or changed at any time throughout the placement as deemed necessary but generally they are updated at least every six months in progress team meetings involving all members of the support team. ![]() |
